On receipt of your Payment Authority the Union will commence deductions as soon as possible and in line with your current payment schedule after processing your request.
The Union’s fees are revised yearly. If there are any changes to the Union fees, we will advise you of this giving at least thirty (30) days notice on the Union’s website and via email.
I authorise the Australian Nursing & Midwifery Federation NT (User ID Number 060174) to arrange Member bank details as specified.
Acknowledgement
By submitting this Direct Debit request, I acknowledge having read the Direct Debit Request (DDR) Service agreement and agree to its terms. I authorise and request that this Direct Debit Request remains in force until cancelled, deferred, or otherwise altered and in accordance with the service agreement.
The Union will email you to confirm if your update has been successful and will include your new payment schedule.
I hereby authorise the ANMF NT to debit my credit card as specified.
Ongoing payments will be processed as per your specified schedule.
I hereby authorise the alteration of the amount from the appropriate date in the event of any change in the membership fees. This authority will stand in respect of the above specified credit card issued to me in renewal or replacement thereof, until I notify the ANMF (NT Branch) in writing with two (2) weeks’ notice of the cancellation of this authority.
